BALLARD, Charles "Jukebox Chuck" age 60 of Fairfield, Ohio passed away Tuesday August 28, 2018 at the Tri County Extended Care Center. He was born in Hamilton, Ohio on January 23, 1958 the son of Charles D. and Barbara (Russell) Ballard.

Survivors include his Mother, Barbara Hickey; a daughter, Randi (Jeremy) Hilsercop, two grandchildren, Madison and Michael Hilsercop; Three sisters, Toni Shafer, Bobbi Lowe, and Beth Hannah; two brothers, Billy and Matthew Hickey.

He was preceded in death by his father.
