CURTIS, Donald R. D.D.S. age 58 formerly of Fairfield and Toledo, Ohio currently residing in Phoenix, Arizona died Wednesday, November 21, 2012, at Hospice of the Valley in Surprise, Arizona. He was born in Hamilton, Ohio on December 30, 1953, the son of James E. and Elaine L. (Zimmerman) Curtis and was a 1972 graduate of Badin High School and a 1978 graduate of The Ohio State University College of Dentistry. Dr. Curtis was a dentist in Toledo for twenty-five years.

Survivors include a son, Justin R. Curtis of Phoenix, Arizona; a daughter, Tracy Curtis of New York City, NY; two brothers, Robert (Ann) Curtis of Torrance, CA and Dave (Terry) Curtis of Midlothian, VA; a sister, Cathy (Dr. Tom) Green of Cincinnati, OH.

He was preceded in death by his parents.

Visitation will be from 9-11:00 am Saturday in the Zettler Funeral Home, 2646 Pleasant Ave. Funeral services will be at 11:00 am Saturday in the funeral home with Fr.
